Job summary

Honest Health in Albuquerque, NM is seeking a Population Health Manager to lead partnerships with physician practices and advance value-based care. This remote role requires travel up to 70% to visit practices, build relationships, review performance data, and guide teams toward quality, cost and patient outcomes.

You will work with PCPs, practice managers, and network partners, onboard practices, analyze scorecards, and translate insights into action plans, helping clinics achieve benchmarks

Qualifications

• Bachelors in healthcare admin or related field preferred; 4+ years in practice management or IPA work.
• 5+ years’ healthcare experience; proven engagement with PCPs and health systems.
• Strong knowledge of clinic ops, medical office management, and staff supervision.

Responsibilities

• Build and maintain relationships with clinical partners across networks.
• Provide outreach to practices via in-person, telephonic, and electronic methods.
• Onboard practices to Honest services and drive program adoption.
• Educate practices on value-based care and drive cost efficiencies and outcomes.
