Honest Health is seeking a Physician Practice Partner to strengthen partnerships with PCPs and practices, guiding them toward value-based care while aligning with Honest's quadruple aim.

The role is remote, based near Buffalo, NY, with up to 70% travel to local practices; you will deliver performance insights, scorecards, and best-practice guidance to improve quality, costs, and patient outcomes.

Qualifications

• Bachelor’s degree preferred; 4+ years in practice management or physician leadership.
• Master’s degree is a plus.
• 5+ years in healthcare environment with engagement of PCPs, specialists, and health systems.
• Strong knowledge of value-based care, coding, quality, and billing.
• Able to travel up to 70% to local practices and conduct remote work efficiently.
• Excellent verbal, written, and interpersonal communication.

Responsibilities

• Build and maintain relationships with clinical partners and stakeholders.
• Provide onboarding and operational guidance for Honest services.
• Educate practices on value-based care opportunities.
• Analyze performance data and develop improvement strategies.
• Deliver scorecards and help practices meet KPIs.
• Collaborate with Honest Health’s clinical team to support care team objectives.
• Maintain documentation and CRM inputs for provider engagement.
• Prepare reports for the Director of Market Operations.
