Job summary

Moms In Motion in Virginia is hiring Service Facilitator (SF) Case Manager. You’ll visit families in their homes, helping them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT) and support independent, safe living.

We’re looking for compassionate, organized individuals with at least 2 years of experience supporting people with disabilities or the elderly. No degree required; a valid driver’s license and reliable vehicle are essential. Remote work from your laptop is supported.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; emphasis on heart and experience.
• Valid driver’s license and reliable vehicle.
• Ability to pass background check and provide 2 professional references.
• Strong written and verbal communication skills.
• Bonus: Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Be the go-to guide for families navigating VA waiver programs.
• Write up plans of care and assessments that lead to better outcomes.
• Troubleshoot challenges like authorizations and timesheets.
• Build lasting relationships with families built on respect and trust.
• Work remotely from your laptop/tablet.
