Moms In Motion is seeking a Service Facilitator (SF) Case Manager to support Virginia families by navigating Medicaid Waiver programs, visiting clients at home, and coordinating plans of care. This role requires living in VA with local travel to client homes and the ability to work remotely from a laptop.

No degree is required; 2+ years experience with disabilities or elderly care and strong communication skills are valued.

Competenze

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; emphasis on heart and experience.

Mansioni

• Visit clients in their homes locally to provide support.
• Serve as the go-to guide for families navigating waiver programs.
• Draft care plans and assessments that drive meaningful outcomes.
• Troubleshoot service authorizations, timesheets, and related processes.
• Build lasting, trusted relationships with families.
• Work remotely from a laptop/tablet as part of the role.
