Job summary

Moms In Motion / At Home Your Way seeks a Service Facilitator (SF) Case Manager in Virginia. You will visit clients’ homes locally, guiding families through Medicaid Waiver programs (CCC+, CL & FIS, EPSDT) and write care plans that improve health and independence.

Ideal candidates have strong communication, are comfortable with technology, and can manage timesheets and authorizations. A valid driver's license and reliable vehicle are required; the role supports remote work from a laptop or

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required per the job description.
• Must have a valid driver’s license and a reliable vehicle.
• Technology proficient for email, documents, spreadsheets, portal navigation and video calls.
• Able to pass a background check and provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot issues like service authorizations and timesheets.
• Build lasting relationships with families founded on respect, patience and trust.
• Work remotely from your laptop/tablet.
