Service Facilitator (SF) Case Manager

Moms In Motion is seeking a Service Facilitator to visit families in their homes across Virginia, helping them navigate CCC+, CL & FIS, and EPSDT waiver programs. You will write care plans, address service authorizations, and build trustful relationships while supporting independence and safety in the community.

Ideal candidates bring at least 2 years of relevant experience, are tech-savvy, hold a valid driver’s license, and can pass a background check.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter more than credentials.
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver's license and reliable vehicle (no client transport).
• Strong written and verbal communication skills.
• Able to pass a background check and provide 2 professional references.
• Bonus if you have Person-Centered Thinking/Planning training.

Responsabilités

• Hit the road locally to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that make a difference.
• Troubleshoot challenges like service authorizations and timesheets.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop/tablet.
• Must live in VA and perform daily local travel to client homes.
