Job summary

Moms In Motion in Virginia is seeking a Service Facilitator (SF) Case Manager to connect with families, help navigate Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and support living safely and independently.

You will travel locally to client homes, work remotely from a laptop, and write care plans and assessments while building trust with families. No degree required; at least 2 years of relevant experience and a valid driver’s license are needed.

Qualifications

• Two years of experience supporting individuals with disabilities or the elderly.
• No degree required.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• Must have a valid driver’s license and a reliable vehicle (no client transport).
• Strong written and spoken communication skills.
• Able to pass a background check and provide two professional references.
• Bonus: Person-Centered Thinking/Planning training.

Responsibilities

• Visit clients in their homes locally.
• Guide families navigating waiver programs.
• Write care plans and assessments that make a difference.
• Troubleshoot service authorizations, timesheets, and related tasks.
• Build lasting relationships with families based on respect and trust.
• Work remotely from a laptop/tablet.
