Job summary

Upbring in Dallas, TX, seeks a URM Case Manager to provide case management and supportive services for children in foster care within the URM program. You will act as a liaison with referring agencies, following licensing standards and federal requirements to promote resettlement and self-sufficiency.

Responsibilities include conducting monthly visits, maintaining accurate case files, coordinating with foster families and service providers, and responding to needs with compassion and timeliness.

Qualifications

• Bachelor's degree in Social Work or closely related field.
• Three (3) years' relevant experience.
• Bilingual in English and Spanish.
• Must be 21 years or older and able to work evenings and Saturdays.
• Strong ability to establish rapport with children and families.
• Proficient with Word, Excel, PowerPoint and Outlook.

Responsibilities

• Provide case management services to foster care and URM families.
• Conduct monthly home visits to assess mental, emotional, and social well-being.
• Create and maintain URM case files and documentation.
• Complete case notes and maintain logs for all cases.
• Provide on-call services on rotation as needed.
• Collaborate with management to support agency objectives.
• Coordinate with professionals, court officials, and families to address URM needs.
• Respond to URM and foster family concerns with compassion and timeliness.
• Other duties as assigned.
