Human Touch Home Health is seeking a PRN Physical Therapist to serve patients in Alexandria, Virginia. The right candidate will perform in-home evaluations, develop individualized plans of care, and deliver therapy focused on strength, mobility, balance, and independence.

Qualifications

• Active Virginia Physical Therapist License Required.
• Graduate of an accredited Physical Therapy program.
• Valid Driver's License and reliable transportation.
• Strong clinical and communication skills.
• Ability to independently complete home health visits.
• WellSky experience is a plus.
• Alexandria area availability as soon as possible preferred.

Responsibilities

• Complete in-home physical therapy evaluations and develop plans of care.
• Provide gait, balance, transfer, and mobility training.
• Educate patients and caregivers; monitor progress and adjust plans.
• Assess home safety and fall risks; coordinate with physicians and nurses.
• Complete documentation using WellSky and plan discharges.

Human Touch Home Health has an immediate need for a PRN Physical Therapist (PT) to serve patients in Alexandria, Virginia.

We have patients who need care now and are looking for a dependable PT who can help us meet that need.

What You'll Do

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength and functional independence
• Assess home safety and fall risks
• Educate patients and caregivers
• Monitor progress and adjust treatment plans
• Complete reassessments and discharge planning
• Coordinate care with physicians, nurses, therapists, and other team members
• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical and communication skills
• Ability to independently complete home health visits
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
• Ability to accept patients in Alexandria as soon as possible is strongly preferred
