Job summary

Human Touch Home Health is seeking a Physical Therapist (PT) for PRN home health opportunities throughout Washington, DC. You will conduct in-home evaluations, develop individualized plans of care, and deliver therapeutic exercise and mobility training in patients' homes.

Join a team that emphasizes one-on-one care, flexible scheduling, and independence. The role supports ongoing professional growth and requires active DC PT license and ability to work independently in patients' homes.

Qualifications

• Active Washington, DC Physical Therapist License required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and communication skills.
• Dependable transportation to patient visits.
• Ability to work independently in patients' homes.
• Home Health and OASIS experience preferred, but not required.

Responsibilities

• Complete physical therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer, and mobility training.
• Assess home safety and fall risks.
• Educate patients and caregivers.
• Monitor progress and adjust treatment plans.
• Complete progress assessments and discharge planning.
• Coordinate care with physicians, nurses, PTAs, and other team members.
• Complete accurate and timely documentation using WellSky.
