Job summary

Human Touch Home Health is seeking a compassionate Home Health Physical Therapist to join our team serving communities across the DMV. You will provide one-on-one, in-home therapy, design personalized plans, and help patients regain mobility and independence with flexible scheduling.

Qualifications

• Active PT license in DC, MD or VA is required or multi-state licensure.
• Valid driver's license with reliable transportation.
• Strong clinical assessment, communication and documentation skills.
• OASIS experience and WellSky familiarity are a plus.

Responsibilities

• Perform PT evaluations, SOC, reassessments and discharge visits.
• Create individualized care plans based on patient goals.
• Deliver skilled PT services in patients' homes.
• Educate patients and caregivers on exercises, mobility, balance and safety.
• Monitor progress and adapt treatment plans accordingly.
• Collaborate with physicians, nurses and the care team.
• Document accurately using WellSky.

As a Home Health Physical Therapist, you'll provide personalized, one-on-one care that helps patients regain strength, improve mobility, restore balance, and rebuild confidence—all in the comfort of their own homes.

What You'll Do

• Complete physical therapy evaluations, Start of Care (SOC), reassessments, and discharge visits
• Develop individualized plans of care based on patient goals and clinical needs
• Provide skilled physical therapy services in patients' homes
• Educate patients and caregivers on therapeutic exercises, mobility training, balance improvement, fall prevention, and home safety
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with physicians, nurses, social workers, and the interdisciplinary care team
• Complete timely and accurate clinical documentation using WellSky

Qualifications

• Active Physical Therapist License in Washington, DC, Virginia, or Maryland (multi-state licensure is a plus)
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment, Communication, and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience Preferred
• Experience with WellSky Home Health is a plus
• Passion for Delivering Exceptional Patient-Centered Care
