Job summary

Human Touch Home Health in NW Washington, DC, is seeking a DC-licensed Physical Therapist (PT) for immediate home health visits. This PRN role offers flexible scheduling, one-on-one patient care, and a chance to build a caseload around your availability.

You will perform evaluations, develop plans of care, and coordinate with the care team to ensure high-quality, patient-centered outcomes. Ideal candidates are DC-licensed, with strong clinical and communication skills, and able to work

Qualifications

• Active Washington, DC Physical Therapist license required.
• Graduate of an accredited physical therapy program.
• Dependable transportation to patient visits.
• Strong clinical and communication skills.
• Ability to work independently in patients' homes.
• Home Health and OASIS experience preferred but not required.
• WellSky experience is a plus.

Responsibilities

• Complete PT evaluations in patients' homes.
• Develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer and mobility training.
• Assess home safety and fall risk.
• Educate patients and caregivers.
• Monitor progress and update treatment plans.
• Complete progress assessments and discharge planning.
• Coordinate with physicians, nurses and care team.
• Complete timely documentation using WellSky.

Human Touch Home Health has an immediate need for a Physical Therapist (PT) to provide home health visits to patients in Northwest Washington, DC.

We have patients who need care now and are looking for a DC-licensed PT who can add visits to their current schedule or build a PRN caseload with our team.

What You'll Do
