Job summary

Human Touch Home Health in Richmond, VA is seeking a compassionate Physical Therapist to join our growing team on a PRN basis, providing skilled services in patients' homes across Richmond, Henrico, Chesterfield, and surrounding communities. You will evaluate, treat, and educate patients while building a caseload that fits your availability.

This role offers flexibility, autonomy, and meaningful impact with one-on-one patient care, mileage reimbursement, and supportive leadership as you help

Qualifications

• Active Virginia Physical Therapist License Required.
• Valid Driver's License and Reliable Transportation.
• Strong Clinical Assessment and Documentation Skills.
• Excellent Communication and Organizational Skills.
• Home Health Experience Preferred but Not Required.
• Experience with OASIS is a plus.

Responsibilities

• Complete evaluations, reassessments, and discharge visits.
• Develop individualized plans of care based on patient goals.
• Provide skilled physical therapy services in patients' homes.
• Educate patients and caregivers on mobility, therapeutic exercises, fall prevention, and home safety.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians and interdisciplinary team members.
• Complete timely and accurate clinical documentation.
