Job summary

Human Touch Home Health in NW DC is seeking a DC-licensed Physical Therapist (PT) to provide home health visits and build a PRN caseload with our team. You will evaluate patients, develop plans of care, and deliver therapeutic exercise, gait, balance, transfer, and mobility training in patients' homes.

The role requires a DC license and the ability to work independently, with opportunities to coordinate with physicians and nurses and to document care using WellSky.

Qualifications

• Active Washington, DC Physical Therapist License Required.
• Graduate of an accredited Physical Therapy program.
• Dependable transportation to patient visits.
• Strong clinical and communication skills.
• Ability to work independently in patients' homes.
• Home Health and OASIS experience preferred, but not required.
• WellSky experience is a plus.

Responsibilities

• Complete PT evaluations in patients' homes.
• Develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer, and mobility training.
• Assess home safety and fall risks.
• Educate patients and caregivers.
• Monitor patient progress and update treatment plans.
• Complete progress assessments and discharge planning.
• Coordinate with physicians, nurses, and other members of the care team.
• Complete timely documentation using WellSky.

Human Touch Home Health has an immediate need for a Physical Therapist (PT) to provide home health visits to patients in Northwest Washington, DC.

We have patients who need care now and are looking for a DC-licensed PT who can add visits to their current schedule or build a PRN caseload with our team.
