Job summary

Memorial Home Services is seeking an Urgent Care Advanced Practice Provider in Springfield, IL. The role provides care across all ages, supervising physician collaboration, and adherence to evidence-based standards. Day-to-day includes patient evaluation, documentation, and coordinated care plans.

Licensed APRN or PA with current CPR; ACLS where required. This full-time position involves three 12-hour shifts weekly and every third weekend rotation.

Qualifications

• Must possess and maintain licensing as an APRN or PA as required by Illinois State Law.
• Valid CPR certification required.
• ACLS where required.

Responsibilities

• Assess, diagnose, and treat patients in collaboration with supervising physician.
• Document patient visits and testing for review and signature; communicate with physician before costly tests or referrals.
• Complete timely documentation per Medicare/Medicaid standards and MHS policies.
• Coordinate resources required by the patient; collaborate with other health care professionals as needed.
• Develop, implement, and evaluate plans for adherence to evidence-based quality indicators.
• Assist with implementing research findings and monitoring impact on patient care outcomes.
