Job summary

Koke Mill Family Medicine in Springfield, IL, invites a Nurse Practitioner or Physician Assistant to join as a Full Time Urgent Care Advanced Practice Provider. You will treat patients of all ages across the lifespan, under physician direction, and coordinate care with a team to ensure high-quality outcomes.

Licensure as an APRN or PA with appropriate certifications is required, along with CPR and possibly ACLS.

Qualifications

• APRn or PA licensure required per Illinois state law.
• Maintain DEA license where required. CPR certification required; ACLS where required.
• 1+ year post-graduate experience as a mid-level provider preferred.

Responsibilities

• Assess, diagnose, and treat patients in collaboration with supervising physician within clinical scope.
• Document visits and testing for physician review and signature; communicate with physicians as needed.
• Complete timely documentation for Medicare/ Medicaid and payer guidelines.
• Coordinate patient resources and care plans; collaborate with other healthcare professionals and family counselors.
• Develop and evaluate plans for adherence to quality indicators; act as clinical consultant to staff.
• Review current literature and implement findings to improve patient outcomes.
• Utilize data to analyze performance and identify productivity or cost improvements.
• Support specialty development and educational activities; participate in community programs.
• Assist in achieving revenue/expense targets and maintain planned volumes; support patient satisfaction goals.
