Job summary

Memorial Health in Springfield, IL is seeking a Nurse Practitioner or Physician Assistant for a full-time, on-site Primary Care Advanced Practice Provider position. The role focuses on patient assessment, diagnosis, and treatment under physician supervision with adherence to standard protocols and comprehensive documentation.

Compensation is hourly, with additional incentive opportunities. Requires APRN or PA licensure in Illinois, CPR (and ACLS if required), and 1+ year post-graduate experience

Qualifications

• APRN or PA licensure as required by Illinois law
• DEA license requirements if applicable
• CPR certification required
• ACLS as required
• 1+ year post-graduate experience preferred

Responsibilities

• Assess, diagnose, and treat patients in collaboration with supervising physicians.
• Document physician visits and testing for review and signature.
• Communicate with physicians for clarifications before ordering tests or referrals.
• Maintain timely and accurate medical records per payer and policy standards.
• Coordinate resources and collaborate with health care professionals to develop and implement care plans.
• Evaluate and modify care plans to improve outcomes.
• Contribute to education, research, and community involvement as needed.
