Job summary

Scotland Health Care System in Pembroke, North Carolina, seeks a Family Practice FNP/PA to provide primary care under physician supervision, interview patients, perform history and exams, manage chronic conditions, and coordinate care within approved protocols.

You will develop care plans, prescribe and adjust medications as allowed, order tests, refer to specialists, educate patients, and participate in community health activities and continuing education.

Responsibilities

• Interprets clinic operating procedures and ensures compliance with standards of care.
• Interviews patients and takes a complete medical history during initial visit.
• Performs comprehensive and problem-focused history and physical examinations; provides routine care.
• Maintains the H&P, problem list, medication list, and other assessment instruments.
• Provides primary care under the direction of the supervising physician.
• Formulates diagnoses and plans within established protocols with supervision.
• Prescribes and adjusts medications according to laws and clinic protocols.
• Prepares requisitions for tests and assists with patient preparation.
• Provides ongoing evaluation and management for patients with multiple chronic conditions.
• Initiates referrals to specialists or community resources as needed.
• Provides health education and counseling to patients and families.
• Maintains current skills through continuing education activities.
• Participates in Scotland Free Clinic and hospital events.
• Complies with JCAHO audits by maintaining 90% or above on chart audits.
