American Family Care, Inc. is seeking an Advanced Practice Provider (APP) to care for patients in their location in York County, Hanover. The APP will perform detailed health histories, develop medical assessments, and provide guidance on patient care. This role requires strong communication skills and relevant licensure.

Qualifications

• 1 year ER/UC experience preferred.
• DEA number and state-controlled substance certificate required.
• Able to work independently.

Responsibilities

• Perform complete health histories and develop comprehensive medical assessments.
• Formulate medical and nursing diagnoses and institute appropriate therapies.
• Institute emergency measures in critical situations.
• Interpret patient data to determine management and treatment.
• Provide health care guidance to patients and families.
