Outcome HC is seeking an RN or LPN licensed in New Jersey to oversee a designated unit, perform assessments under supervision, distribute medications, document treatments, and supervise CNAs.

The role emphasizes SBAR communication, resident involvement in care when possible, accurate handoffs, and collaboration with multidisciplinary teams to ensure continuity of care. The incumbent must follow standard precautions, protect Resident's Rights, and adhere to facility policies.

Qualifications

• Must be licensed RN or LPN in NJ.
• Current CPR certification or within 90 days of hire.
• Other certifications may be required for other areas of the organization.

Responsibilities

• Uses dual identifiers to confirm resident identity prior to medications and treatments.
• Practices SBAR communication when interacting with healthcare team.
• Encourages residents to participate in their own care when possible.
• Receives detailed handoffs from outgoing nurses for proper follow-up.
• Recognizes changes in resident condition and documents appropriately.
• Initiates and updates Plan of Care on admission and as needed.
• Collaborates with multidisciplinary teams to assure continuity of care.
• Adheres to Standard Precautions and infection prevention strategies.
• Ensures compliance with Residents' Rights and facility policies.
• Transcribes orders accurately and communicates with Pharmacy for timely delivery.
• Maintains safe environment and reports unsafe situations.
• Follows chain of command and proper hand washing technique.
• Administers medications according to policy and procedure.
• Notifies responsible party of changes in patient condition.
• Works with Social Worker to address resident needs.
• Checks syringes and counts narcotics at end of shift.
• Orders stock medications as needed.
• Admits and discharges residents per facility procedures.

Education

RN or LPN license in NJ
