Job summary

Outcome HC is seeking a Registered Nurse (RN) in New Jersey to manage unit-based patient care, including assessments, medication administration, and clinical documentation under supervision. The RN will supervise CNAs and other staff while ensuring safety and quality of care.

The role requires licensure as an RN in NJ, current CPR certification, and compliance with policy-driven procedures and interdisciplinary collaboration. Strong communication and patient-centered care are essential.

Qualifications

• Graduated from an accredited school of nursing and licensed by the state of NJ as an RN.
• Current CPR or certified within 90 days of hire
• Other certifications may be required for other areas of the organization

Responsibilities

• Uses dual identifiers to confirm resident/guest identity prior to medication administration and treatments.
• Practices SBAR for communication with healthcare teammates.
• Encourages resident/guest involvement in their own care when possible.
• Receives and applies detailed report from outgoing nurses for proper handoff.
• Recognizes changes in patient condition and documents appropriately.
• Initiates Plan of Care on admission and updates as needed.
• Interacts with multidisciplinary teams to assure continuum of care.
• Adheres to infection prevention and standard precautions.
• Transcribes orders accurately on MAR/TAR and relays to pharmacy.
