Job summary

Outcome HC in New Jersey is seeking a Licensed Practical Nurse (LPN) to oversee a designated unit under RN supervision. The role includes assessment, medication distribution, treatment documentation, and supervision of CNAs and other staff.

The LPN will use SBAR for communication, participate in care planning, and coordinate with multidisciplinary teams to ensure a seamless continuum of care while adhering to infection control and resident rights.

Qualifications

• Graduated of an accredited school of nursing.
• Current CPR or certified within 90 days of hire.
• Other certifications may be required for other areas of the organization.

Responsibilities

• Uses dual identifiers to confirm resident/guest identity prior to medication administration and treatments.
• Practices SBAR (situation, background, assessment, recommendation) in communications.
• Encourages resident/guest involvement in their own care when possible.
• Receives full and detailed report from outgoing nurses for proper communication and follow-up.
• Recognizes and responds to changes in resident’s condition and documents appropriately.
• Initiates Plan of Care on admission and updates plans as needed.
• Interacts with multidisciplinary teams to assure continuum of care.
• Practices Standard Precautions and adheres to infection prevention strategies.
• Demonstrates compliance to Resident’s Rights.
• Transcribes physician orders correctly on MAR & TAR.
• Faxes POS to Pharmacy to ensure timely delivery and administration.
