Job summary

Qualifications

Responsibilities

• Provide quality nursing care to patients with life-limiting illness.
• Participate with interdisciplinary team to develop care plans.
• Educate patients and families to maintain skills and independence.
• Document care accurately and timely in patient charts.
• Participate in Quality Improvement to enhance patient care.
• Supervise Patient Care Partners, unit receptionists and volunteers.
• Complete daily acuity assessments for assigned patients.
• Attend to patient and family at time of death.
