HHSC is seeking a Treatment Team Coordinator to support treatment planning by gathering input from disciplines and compiling the Person-Centered Recovery Plan (PCRP). The role operates under general supervision with some initiative and independent judgement and may require overtime or alternate shifts as needed.

Essential duties include data entry into medical records, coordination of treatment meetings, and serving as a liaison among staff, patients, and families.

Formación

• Graduation from standard senior high school or equivalent.
• At least two (2) years of experience in a professional support role.
• Must possess a valid Texas driver license or obtain it within 90 days of hire.

Responsabilidades

• Attend work regularly and adhere to shift schedules including rotation, extended shifts, and overtime as needed.
• Collect, synthesize, transcribe, and enter data into the medical record to document treatment recommendations.
• Enter information into the EMR reflecting treatment goals, interventions, and staff assignments.
• Develop and maintain a tracking system to ensure treatment/recovery plans are completed timely.
• Coordinate, schedule, and attend treatment team meetings; maintain electronic and paper files.
