Job summary

Copa Health is seeking a Care Treatment Coordinator to facilitate care coordination for members across health plans. You’ll ensure services are implemented per contract requirements and maintain up-to-date documentation while collaborating with behavioral health, medical and wellness providers to support members’ recovery goals.

Qualifications

• Bachelor’s degree in behavioral health related field and 2 years’ experience in behavioral health.
• Bachelor’s degree in non-behavioral health field with at least 4 years’ behavioral health experience; or High school diploma with 6 years’ behavioral health experience.
• Must have Level 1 fingerprint clearance card or eligible to receive within eight weeks of hire.
• Valid AZ Driver’s License and vehicle with insurance; able to meet driving requirements.

Responsibilities

• Accept referrals and coordinate care with psychiatric, primary care, behavioral health, medical and dental providers to ensure ISP services are implemented.
• Update and maintain required medical record documentation.
• Maintain an abbreviated Case Manager caseload (Supportive/Connective Level of Care).
• Complete an Integrated Comprehensive Assessment (ICA) annually as required by the health plan and update it as necessary.
• Coordinate care with all health plan liaisons.
• Develop discharge and aftercare plans prior to discontinuation of services.

Education

Bachelor’s degree in behavioral health related field

High school diploma or GED with 6 years behavioral health experience
