Job summary

Health & Human Services Comm in San Antonio State Hospital is seeking a Treatment Team Coordinator to join the Administrative team for the San Antonio State Hospital campus. The role supports treatment planning meetings, gathers input from all disciplines, and drafts Person-Centered Recovery Plans (PCRP) to guide patient care.

You will coordinate scheduling, maintain files, and ensure timely updates in the electronic medical record.

Qualifications

• Two years of experience in a professional support role.
• Knowledge of HIPPA and confidentiality rules.
• Knowledge of the Electronic Medical Record (EMR).
• Proficient knowledge of computer use and psychiatric terminology.
• Ability to synthesize large amounts of information into clearly written documents.
• Ability to coordinate team members and finalize plans according to policy.

Responsibilities

• Attends work regularly and may follow a specific or rotating shift schedule.
• Collects, analyzes, and enters data into the medical record for treatment planning.
• Completes data entry into the EMR with accurate treatment goals and staff assignments.
• Develops and maintains tracking to ensure timely completion of PCRPs.
• Coordinates treatment team meetings and maintains files (electronic and paper).
• Performs other duties and may participate in disaster recovery or COOP activations.
