Job summary

Texas Health and Human Services Commission is seeking a Treatment Team Coordinator at the Waco Center for Youth. The role focuses on building and updating the Person-Centered Recovery Plan (PCRP) and supporting treatment planning by coordinating multidisciplinary input and maintaining comprehensive records.

Responsibilities include data entry into the EMR, organizing treatment team meetings, and ensuring plans are completed accurately and on time.

Qualifications

• Knowledge of office practices and administrative procedures.
• Knowledge of HIPPA and confidentiality rules.
• Knowledge of the Electronic Medical Record (EMR).
• Proficient knowledge and skill in computer use and the written language including spelling, punctuation, and medical, as well as psychiatric, terminology.
• Ability to synthesize large amounts of information into a clearly written and concise document.
• Ability to coordinate team members so that plans and reviews are finalized according to policy.
• Ability to communicate effectively using both oral and written methods of communication.

Responsibilities

• Attends work on a regular basis and may be required to work a specific shift schedule or rotating schedule; performs other duties as assigned.
• Collects, synthesizes, transcribes, and enters data into the medical record documenting treatment recommendations into a comprehensive individualized treatment/recovery plan.
• Completes data entry into the medical record reflecting treatment goals, interventions, and staff assignments.
• Develops and maintains a tracking system to ensure treatment/recovery plans are completed timely and updated as needed.
• Performs administrative duties including coordinating, scheduling, and attending treatment team meetings; maintains files.
