The Wellstone Group is seeking a full-time LPN or RN Treatment Nurse to join our care team in a physical facility setting. The role focuses on resident observation, documentation, wound care, and collaboration with the interdisciplinary team to deliver individualized care plans.

Responsibilities

• Observe, assess, and document resident conditions and changes.
• Perform skin assessments according to facility procedures.
• Implement interventions to prevent pressure ulcers and educate staff, residents, and families.
• Administer treatments and medications per physicians' orders and guidelines.
• Manage skin tears, wounds, and related conditions.
• Collaborate with the interdisciplinary team to develop individualized care plans.
• Ensure accurate and timely documentation.
• Support infection control surveillance and adhere to guidelines.
• Monitor subordinate staff activities and assist in incident investigations.
• Identify and secure equipment and supplies for wound care.

Responsibilities of the Treatment Nurse include:
