Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations, traveling to patients' homes. You’ll conduct 45-minute visits with medical history review, physical exams, and optional diagnostic tests as needed.

Clinicians use an iPad and clinical workflow; you may travel up to 100% across designated areas, and must hold active licenses in coverage states. This role offers comprehensive benefits and competitive compensation.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses as required.

Responsibilities

• Provide In-Home Health Evaluations.
• Travel across defined coverage area up to 100%.
• Conduct 45-minute visits including medication and medical history review and physical evaluation.
• Use iPad and clinical workflow during evaluations; may perform diagnostic tests if ordered by health plan.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits as needed.
