Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to perform In-Home Health Evaluations across a defined coverage area. The role involves travel up to 100% of the time, visiting members at their homes, reviewing medications and medical history, performing physical exams, and coordinating with health plans.

You will use a tablet-based workflow, complete evaluations in about 45 minutes, and may conduct virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.
• MD or DO with active license(s) to practice medicine.

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
• Be flexible to travel locally and within licensed states.
