Job summary

Elara Caring is seeking a licensed Speech-Language Pathologist to provide in-home services in Maine. You will evaluate patients, develop and implement therapy plans of care, and coordinate with physicians and the care team to ensure quality, patient-centered outcomes.

This role requires travel up to 50% across the branch coverage area, compliance with OASIS and PPS guidelines, thorough documentation, and training patients and caregivers in home therapy programs.

Qualifications

• Master’s degree in Language Pathology from an accredited program.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of clinical SLP experience; travel 50% required.

Responsibilities

• Provide home-based speech-language pathology services.
• Develop and implement therapy plans of care.
• Evaluate patients and determine rehabilitation needs.
• Admit patients and assess needs per OASIS and PPS.
• Instruct patients and caregivers in home therapy programs.
• Document therapy notes and Plans of Care.
