Job summary

Elara Caring is seeking a Speech-Language Pathologist to deliver high-quality, home-based therapy across our care areas. You will evaluate patients, develop plans of care, and provide direct services in the patient’s home, reporting to the Branch Director or Clinical Supervisor.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of experience as an SLP in a clinical setting.
• Able and willing to travel within branch/office coverage area.
• Ability to frequently lift, push, pull, and move up to 50 pounds.
• Must have dependable vehicle, valid driver's license, and auto insurance.
• Willing to travel up to 50% of the time.

Responsibilities

• Promotes Elara Caring’s philosophy and care standards.
• Evaluates patient rehabilitation needs and develops a care plan.
• Performs initial and ongoing speech-language evaluations and re-evaluations.
• Initiates patient safety planning with family involvement.
• Communicates findings to supervisor and care team.
• Documents plans and visits per policy; coordinates with physicians.
• Provides direct speech-language pathology services in home settings.
