Job summary

Elara Caring is hiring a Speech-Language Pathologist to deliver in-home therapy across our service areas. You will evaluate patients, develop and implement individualized care plans, and collaborate with physicians and the care team.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of experience as an SLP in a clinical care setting.
• Ability and willingness to travel within branch/office coverage area.
• Must be able to lift up to 50 pounds and assist with patient transfers.

Responsibilities

• Evaluates patient needs and develops a plan of care according to findings.
• Admits patients and assesses speech-language therapy needs according to regulatory guidelines.
• Communicates findings to supervisor and care team.
• Implements and documents Plan of Care and coordinates with physician orders.
• Provides direct Speech-Language pathology services per standards of practice.
