Job summary

Healing Partners is seeking a Wound Care Nurse Practitioner to lead health care for residents across multiple Skilled Nursing Facilities. You will travel daily to facilities, evaluate and treat wound-related conditions, interpret tests, and coordinate with physicians and the medical director to ensure high-quality patient care.

The role requires an active RN license, APRN certification, and a Master’s in Nursing.

Qualifications

Covid vaccination proof required.
Current RN license in the state.
APRN license or ability to obtain prior to hire.
Completion of an approved nurse practitioner program.
Board of Nursing approval to perform medical acts.
Master’s degree in Nursing.
Excellent organizational skills and attention to detail.
Strong analytical and problem-solving skills.

Responsibilities

Plan, direct and evaluate resident and employee health care in long-term care.
Daily travel to various facilities; scheduling varies by region.
Examine and treat chronic and acute wound-related illnesses and injuries.
Interpret diagnostic tests and recommend follow-up actions.
Refer to and update physicians; write prescriptions; perform bedside procedures.
Collaborate with medical director on resident and staff care.
