Job summary

Healing Partners is seeking a Wound Care Nurse Practitioner to lead and coordinate wound care across multiple Skilled Nursing Facilities in the local region. This full-time, day-shift role requires daily travel with schedules varying by site, and a focus on improving resident outcomes through advanced wound management.

You will evaluate patients, interpret tests, prescribe when appropriate, and work with the medical director to guide care. A valid APRN license and nursing license are required.

Qualifications

• Proof of Covid vaccination.
• Currently licensed as a registered nurse in the state.
• Active APRN license or ability to obtain prior to hire.
• Successful completion of an approved nurse practitioner program.
• Current approval by the Board of Nursing to perform medical acts.
• Master’s degree of Nursing.
• Excellent organizational skills and attention to detail.
• Strong analytical and problem-solving skills.

Responsibilities

• Plan, direct and evaluate resident and employee health care in long-term care.
• Daily travel to various facilities. Regions can vary based on scheduling needs.
• Examine and treat chronic and acute wound-related illnesses and injuries.
• Interpretation of diagnostic tests and recommendation of follow-up.
• Refer to and update physicians, write prescriptions, and perform bedside procedures associated with wound care.
• Work with the medical director in the care and treatment of residents and staff.
• Debridement of wounds by sharp, enzymatic, autolytic, or surgical methods as needed.
• Application and management of topical wound care and associated products.
• Direction and recommendation of associated services to improve wound healing outcomes.
• Educate patients and Primary Care Physicians on current wound management practices.
