Job summary

Healing Partners seeks a Wound Care Nurse Practitioner to lead wound care across multiple Skilled Nursing Facilities. You will assess and manage resident health, travel to sites, and coordinate with physicians and the medical director to optimize wound care outcomes.

The role requires an NP with an active state APRN license, CNAs, and a strong nursing background, with a focus on wound management, patient education, and to ensure safe bedside procedures across facilities.

Qualifications

• Master’s degree in Nursing required.
• Active APRN license in state or ability to obtain before hire.
• Proof of Covid vaccination.
• Excellent organizational and analytical/problem-solving skills.

Responsibilities

• Plan, direct and evaluate resident and employee health care in long-term care.
• Travel daily to multiple facilities; regions vary based on scheduling needs.
• Examine and treat wound-related illnesses and injuries; perform follow-up.
• Interpret diagnostic tests and refer to physicians as needed.
• Educate patients and primary care physicians on wound management practices.
