American Traveler, LLC. is hiring a Home Health RN with Oregon RN licensure to perform Start of Care visits in patients' homes. The role requires OASIS experience and proficiency with HCHB, with a 5x8 schedule and on-call rotation.

Travelers are accepted while each traveler is assigned their own territory; orientation includes billable training modules and timely documentation. A valid driver's license is required and benefits include health insurance, 401(k), and CEUs.

Qualifications

• Active OR RN license in-hand at time of consideration.
• Minimum of 1 year of relevant experience.
• OASIS experience required for SOC visits.
• Proficiency in HCHB charting system required.
• Active driver's license and personal transportation required.
• Hospice/Home Health Checklist completion required.

Responsabilités

• Provide skilled home health nursing care to patients in their residences.
• Home Health setting providing in-home patient care.
• Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM.
• On-call rotation may be required.
• Travel pairs accepted, though each traveler is assigned their own territory.
• Monday orientation required, including completion of billable training modules.
• Timely communication and documentation practices are mandatory.
• RTO (requested time off) must be submitted at the time of application.
• First-time travelers are not considered for this position.
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler.
