Job summary

Qualifications

• Active UT or compact RN license required.
• Current BLS certification (AHA) that does not expire before start date.
• ASN or Nursing diploma required.
• Minimum 1 year of experience in home health, hospice or acute care preferred.
• Valid driver's license with reliable transportation and acceptable driving record.
• Ability to transfer and assist patients independently, including lifting and navigating stairs.
• Basic computer skills and strong verbal and written communication.

Responsibilities

• Home Health setting serving adult and pediatric patients in their home environment.
• Four 10-hour day shifts per week, Monday, Wednesday, Friday, with every other Saturday and Sunday.
• Requires ability to commute to the office twice daily to electronically transfer patient files.
• Uses Epic for charting.
• Assess, plan, implement, and evaluate individualized care plans for home health and hospice patients.
• Coordinate and collaborate with interdisciplinary care teams, including physicians and support staff.
• Deliver care in compliance with hospice/home health policies, legal and regulatory standards.
• Promote patient and family education and prepare patients for transitions in care.
• RTO must be submitted with candidate profile.
• Returning Intermountain employees must be separated from the facility for at least one year before being eligible for a travel assignment.
