American Traveler is seeking a Home Health RN with an active NJ or compact license and at least 2 years of home health experience in a certified agency. The role is based in Morris County, NJ, with a Monday–Friday schedule, 8am–4pm, 37.5 hours weekly, and local travel to patient residences.

The ideal candidate will have OASIS and Homecare Homebase experience, strong wound care and IV therapy skills, and be able to collaborate with the interdisciplinary team to deliver high-quality home health

Qualifikationen

• Active NJ or compact RN license is required.
• Current CPR certification is required.
• 1-2 years RN clinical care experience is required.
• 2+ years home health experience in a certified agency required.
• Associate Degree in Nursing or Nursing Diploma required; BSN preferred.
• CHPN a plus (preferred).
• Homecare Homebase software experience required.
• OASIS experience required.
• Proficient in wound care, IV therapy, venipuncture, and related skills.
• Epic EMR experience is a plus.

Aufgaben

• Provide skilled nursing care to home care/hospice patients in their homes.
• Serve Morris County, NJ, home care unit.
• Work Monday–Friday, 8am–4pm (37.5 hours/week).
• AM shift with no on-call responsibilities.
• Travel locally between patient residences.
• Chart using Epic and Homecare Homebase software.
• Assess patient needs in stable and emergent situations.
• Develop and evaluate individualized plans of care.
• Educate patients and families on care, meds, diet, and safety.
• Supervise home health aides to ensure quality of care.
• Coordinate referrals with interdisciplinary team and physicians.
• Maintain accurate documentation for third-party payers.
• Follow infection control measures to protect staff and patients.
• Local candidates accepted.

• 2+ years of home health experience in a certified home care agency required

• OASIS experience required
• Proficient in wound care, wound vac management, foley catheter and suprapubic insertion, and PICC line care
• IV therapy experience and venipuncture skills required
• Cardiac and respiratory assessment and patient education skills required
• Epic EMR experience a plus

Responsibilities

• Morris County, NJ home care unit
• Monday through Friday schedule, 8am-4pm, 37.5 hours per week

• Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs
• Develop, implement, document, and evaluate individualized Plans of Care
• Provide patient and family education on care, diet, medications, pain management, and home safety
• Supervise and provide clinical direction to home health aides to ensure quality of care
• Coordinate referrals and liaise with interdisciplinary team members, physicians, and other healthcare professionals
• Maintain up-to-date and accurate patient documentation in compliance with third-party payer requirements
• Follow infection control measures to protect staff and patients
