Job summary

American Traveler is hiring a Home Health RN with at least 1 year of home health nursing experience and proficiency in Homecare Homebase EMR.

The role covers visits across University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, and surrounding areas. Expect an 8:30am–5pm schedule, 40 hours weekly, and mileage reimbursement for daily travel.

Qualifications

• Active RN license (NC or Compact)
• BLS certification required
• Valid Driver's License and proof of insurance
• Minimum 1 year of Home Health Nursing experience
• Homecare Homebase EMR charting experience
• Skills include IV therapy, pleurx, wound vac, wound care, patient assessment, disease teaching, and OASIS
• Last 4 digits of SSN and full date of birth required
• Minimum 1 professional reference within past year from a Supervisor/Manager/Director/Charge RN

Responsibilities

• Visit patients across University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, Huntersville and nearby areas
• Drive 60-80 miles daily with mileage reimbursed weekly
• Complete 6 patient visits per day: routine visits, discharges, ROC, recerts and reassessments
• Manage patients with chronic disease, wound care/vac care, IV therapy and pleurx
• Oversee 25-30 patients in case management
• Work a 8:30am-5pm, 40-hour week; weekend rotation optional
• Call not required but available for interested candidates
• Schedule cycle every 13 weeks; plan visits the day before planned appointments
• Use EPIC Rover (view only) and Focura for discharge instructions and wound photos
• Samsung Note 9 provided for documentation and communication
• Support from leadership, scheduler, Clinical Manager, Clinical Supervisor, and peer team
• Orientation includes New Employee Orientation and 5 days with a preceptor
• Holiday coverage expected unless manager approves time off
• RTO requires manager approval
• Navy blue scrubs required
• Free parking
• Team-oriented department culture
• Must reside at least 50 miles from home unit to be eligible
• First-time travelers considered if experienced in home health
• Local candidates not accepted due to radius rule
• Returning employees must be separated from Advocate Health Southeast facility for at least 1 year

• Orientation includes New Employee Orientation (NEO) and 5 days with a preceptor
