Job summary

Job description

American Traveler is hiring a Home Health RN requiring 1 year of home health experience and Homecare Homebase EMR experience for a 13-week assignment.

Requirements

• Active RN license (NC or Compact) required
• BLS certification required
• Valid driver's license and proof of insurance required
• Minimum 1 year of Home Health experience required
• Homecare Homebase (HCHB) EMR experience required with no exceptions
• Skills required: IV therapy, pleurx, wound vac, wound care, patient assessment, disease teaching, OASIS, and case management
• Minimum 1 professional reference dated within the last year from a Supervisor, Manager, Director, or Charge RN
• Last 4 digits of SSN and full DOB required for consideration

Responsibilities

• Home health nursing services within the community, traveling to patients' homes
• Complete 6 visits per day, including routine visits, discharge, ROC, recert, and reassessment
• Manage a caseload of up to 25-30 patients at once
• Care for patients with chronic diseases requiring wound care, IV therapy, pleurx, and wound vac management
• Travel throughout Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy and surrounding areas, averaging 60-80 miles a day
• Day shift schedule, 0800-1700, 40 hours per week
• Weekend rotation not required unless interested in working weekends
• Call not required, but available for those interested
• EMR: Homecare Homebase (HCHB) required, EPIC Rover (view only), and Focura for document and photo uploads
• Schedule patient visits one day in advance
• Required to complete 30 visit equivalents (VE) per week; cancelled appointments do not count as visit equivalents
• Company phone provided for documentation and communication with patients and providers; no company car provided
• Supported by departmental leadership, a scheduler, clinical manager, clinical supervisor, and clinical team peers
• Orientation includes NEO plus 5 days of unit orientation
• Holiday coverage required for 75% of holidays
• RTO subject to manager approval
• Navy blue scrubs required

• Must reside more than 50 miles from the facility to be eligible for the travel position
• Previous employees must be separated from any AHSE facility for at least 1 year following a permanent position
