Job description

American Traveler is hiring a Hospice RN for a home hospice setting, requiring 2+ years of hospice nursing experience and a valid NJ RN license and driver's license.

Requirements

• Active NJ RN license (or compact)
• BLS certification required
• Valid New Jersey driver's license required
• Minimum 2 years of experience as an RN in a home hospice environment required, 3+ years preferred
• Associate's (ASN) degree or Diploma with NJ RN license required
• Experience using an EMR with excellent critical thinking and communication skills
• Skilled in safe medication management, infection control, and pharmacology
• Strong interdisciplinary communication and patient education skills

Responsibilities

• Home hospice and palliative care setting
• Day shift schedule, 8a-4p, 5 days per week
• Every other weekend required with comp days during the week
• Can expect 4-5 patient visits per day as well as admissions
• Float to different counties as needed, including a sister hospital nearby
• No on-call responsibilities
• Uses Epic EMR for charting
• Perform patient assessments to determine hospice appropriateness, learning needs, and progress toward care goals
• Develop, implement, and revise individualized plans of care in collaboration with the interdisciplinary team (IDT)
• Complete admission assessments and transfers within 24 hours, along with twice-daily data transfers
• Attend IDT meetings every two weeks and monthly staff meetings
• Perform HHA supervisions according to standards
• Maintain a safe environment through infection control practices and medication reconciliation
• Provide orientation and mentorship to new employees and participate in performance improvement activities
