• Report to HCC/DCP Manager and ensure continuity of care from hospital to home.

American Traveler is hiring an RN Home Care Coordinator/Discharge Planner for an on-site hospital role requiring MD or compact RN licensure and prior home care/discharge planning experience.

Requirements

• Active MD or compact RN license required
• Current BLS certification required
• Minimum 2 years of home care, discharge planning, or case management experience required
• Epic EMR experience required
• Comparable facility size experience required based on assignment location

Responsibilities

• On-site position within a hospital's Home Care Group (HCC) department
• Functions as a Hospital Discharge Planner and Homecare Coordinator
• Weekday schedule Monday-Friday 8:30 AM-5:00 PM
• Weekend coverage may be required on Saturday and Sunday from 8:30 AM-4:00 PM
• Uses Epic EMR
• Verify that patients have necessary devices for home care such as walkers and commodes
• Coordinate and set up home care services and connect patients with appropriate community support organizations
• Provide education to patients and families regarding home care needs
• Serve as an on-site home-based care resource and expert to hospital staff involved in discharge planning
• Report to the HCC/DCP Manager
• Facilitate continuity of care for patients transitioning from hospital to home
• Local candidates accepted at the same rate for most locations, except a 60-mile radius rule applies with no local candidates allowed for one specific facility
