Job summary

American Traveler, LLC. is hiring an experienced RN Case Manager for an acute care hospital setting, focusing on discharge planning and inpatient case management.

The role involves collaboration with physicians, utilization managers, social workers, and bedside RNs to ensure timely progression of care, assess readmission risks, and coordinate transitions across the patient journey in a fast-paced environment. Open to travelers with onsite duties and flexible holiday coverage.

Qualifications

• Active CA RN license required.
• Current BLS certification required.
• Minimum 5 years of RN Case Management experience required.
• Inpatient acute hospital Case Management experience required.
• Discharge planning experience required.
• Skilled in care coordination, utilization review, and discharge planning.

Responsibilities

• Acute hospital Case Management department.
• Day shift, 5x8 hour schedule, 8:00 AM to 4:30 PM.
• Every other weekend required.
• Patient ratio of 1:10.
• May be assigned to Emergency Department case management duties.
• Responsible for care coordination and care transitions planning throughout the acute care patient experience.
• Collaborates with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely progression of care.
• Conducts patient assessments to determine readmission risk and need for ancillary services.
• Performs utilization management including level of care reviews and length of stay monitoring.
• Screens for 30-day readmissions and develops transition plans with interdisciplinary team.
• Identifies high utilizers and makes appropriate referrals to community resources.
• Documents in EHR and other electronic systems in compliance with CMS and Joint Commission requirements.
• Must reside within 50-mile radius for local rate eligibility.
• Returning staff must be separated for at least 1 year to return as a traveler.
• Holiday coverage may be required for New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day with only 1 holiday request allowed per contract.
• Maximum RTO of 7 days per contract.
• Re-orientation required only if previously oriented at this facility and separated for 6+ months.
