American Traveler is seeking an RN Home Care Coordinator/Discharge Planner for an on-site hospital role in Maryland. The ideal candidate holds an MD or compact RN license and has experience in home care/discharge planning, with Epic EMR proficiency.

The position requires weekday hours 8:30 AM-5:00 PM and potential weekend coverage. Compensation includes comprehensive benefits and travel/reimbursement options for eligible assignments.

Qualifications

• Active MD or compact RN license required.
• Current BLS certification required.
• Minimum 2 years of home care, discharge planning, or case management experience required.
• Epic EMR experience required.

Responsibilities

• Work as an on-site Hospital Discharge Planner and Homecare Coordinator.
• Coordinate and set up home care services for discharge planning.
• Educate patients and families on home care needs and equipment.
• Verify devices for home care (walkers, commodes).
• Report to HCC/DCP Manager and ensure continuity of care from hospital to home.
