Job summary

HealthForce Healthcare seeks an experienced Travel Registered Nurse (RN) for a Long Term Care setting in Dubuque, IA. This role covers 12-hour day shifts and focuses on chronic disease management for frail older adults in a skilled nursing facility.

The nurse will perform physical assessments, administer medications, and coordinate care with a multidisciplinary team. Strong EHR documentation and patient-family education are essential.

Qualifications

• Active RN license in Iowa or an active Compact/NLC RN license is required.
• BLS certification is required or if allowed by facility standards.
• Experience in long-term care (LTC) or skilled nursing facility (SNF) settings is required.

Responsibilities

• Perform comprehensive physical assessments and monitor residents for acute changes.
• Administer medications and manage IV therapy per orders and protocols.
• Provide routine wound care and skin integrity management, including pressure ulcer prevention.
• Document care, treatments, and responses in the EHR/EMR system.
• Collaborate with an interdisciplinary team to create and adjust care plans.
• Educate residents and families on disease management and safety.
• Facilitate transitions of care within the facility or to higher-level care.
