American Traveler, LLC. is hiring an RN Case Manager for a hybrid role across inpatient and community settings, requiring a CA RN license and inpatient case management experience. The position emphasizes complex care coordination and travel-ready candidates.

Provides support across Emeryville and San Francisco facilities with a 2 days onsite / 3 days remote schedule, using Epic, and a 1:25 caseload. Local candidates at local rate; 1-year separation for travelers applies.

Qualifications

• Active CA RN license is required.
• BLS certification required (AHA or ARC).
• Minimum 2 years inpatient case management experience.
• Teaching hospital and community hospital experience preferred.
• Prior travel experience preferred.
• Familiar with utilization review, discharge planning and care coordination.
• Knowledge of InterQual criteria, CMS guidelines, DRG, HIPAA and Joint Commission standards.

Responsabilités

• Supports complex case management across the health system and virtually in the community.
• May float between Emeryville and San Francisco offices as needed.
• Hybrid schedule with 2 days onsite and 3 days remote per week.
• Day shift schedule, 5x8-hour shifts from 0800-1630.
• Uses Epic charting system.
• Case load ratio of 1:25.
• Admission criteria reviews, concurrent and continued stay reviews, discharge planning, disease management, prior authorizations and retrospective reviews.
• Coordinates needs assessments and durable medical equipment orders.
• Must reside within a 50-mile radius for local rate qualification.
• Returning staff must have a 1-year separation before returning as a traveler.
• Maximum RTO of 7 days per contract; 1 holiday request permitted per contract.
• Holidays covered include New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day.
• Labs must be completed within 3–5 days of contract signing.
• Can only be submitted to one Sutter Health facility at a time.
