The position emphasizes collaboration with care teams, detailed patient assessments, and coordinated post-discharge planning using Epic EMR. The role follows a weekday schedule with no weekends or on-call requirements, and candidates must live outside a 75-mile radius of the facility.

Qualifications

• Active RN license (NH or Compact) required.
• BSN required.
• Certified Case Manager (CCM) certification required.
• Minimum 3 years of inpatient clinical experience.
• At least 1 year of Care Management experience.
• Recent Level 1 or comparable Level 2 Trauma experience within 2 years.
• Epic EMR experience preferred.
• BLS certification required.

Responsibilities

• Inpatient hospital setting within the Care Management department.
• Oversee a daily caseload of inpatient patients with varied care needs.
• Attend interdisciplinary rounds and complete initial patient assessments.
• Collaborate with patients, families, caregivers, and the healthcare team to develop plans of care.
• Coordinate post-discharge needs such as home health and inpatient rehab with post-acute referral sources.
• Document and place orders for home health, DME, and nutritional needs within Epic EMR.
• Day shift schedule, Monday through Friday, 8:00 AM to 4:40 PM.
• No weekend or on-call requirements.
• Candidates must reside outside a 75-mile radius of the facility to be considered.
