Job summary

American Traveler is seeking an experienced Case Management RN for a day shift with weekend rotation in Colorado.

You will handle discharge planning, coordinate care, and collaborate with a team of case managers and social workers while using Meditech and CarePort EMR.

Candidates must reside more than 50 miles away; travel perks, housing allowance, and per diem are included.

Qualifications

• Active RN license CO or Compact.
• Current BLS certification.
• Minimum 1 year of recent Case Management RN experience.
• No employment gaps of 90 days or more within the past 12 months.

Responsibilities

• Case Management department with units of approximately 20 beds each.
• Caseload ratios of approximately 1:20 for RNs and 1:40 for social workers.
• Day shift schedule, 8:00 AM to 4:30 PM, with weekend rotation.
• Uses mobile devices and computers for charting.
• EMR systems: Meditech and CarePort.
• Conduct discharge planning assessments and coordinate discharge planning for patients.
• Collaborate with a team of case managers and social workers to manage patient caseloads.
• Adaptability to work across different units is valued.
• Strong communication skills and a professional, friendly demeanor are preferred.
• Candidates must reside more than 50 miles from the facility; no local candidates will be considered.
