Job summary

Healthforce Healthcare LLC is seeking a Physical Therapy Assistant for a LTAC/LTC setting in Winchester, NH. This travel assignment runs 15 weeks beginning 09/28/2026 with guaranteed 35 hours per week and a 5x8 shift (08:00–16:30).

You will support licensed physical therapists, assist with assessments, therapeutic exercises and gait training, and help with discharge planning and family education. NH licensure or eligibility is required; strong communication and documentation skills are essential.

Qualifications

• Active PTA license in NH or eligibility for licensure in NH.
• Graduation from an accredited PTA program.
• Ability to work in a LTAC/LTC setting.
• Strong communication, teamwork and documentation skills.
• Commitment to a 15-week assignment with 35 guaranteed hours per week.

Responsibilities

• Provide PTA support and assist licensed therapists per care plan for LTAC/LTC residents.
• Assist with patient assessments, data collection, therapeutic exercises and gait training.
• Implement modalities and monitor patient responses under supervision.
• Document progress, track goals and contribute to discharge planning.
• Maintain safety, infection control, equipment setup and patient positioning.
• Coordinate with PT, nursing staff and aides; participate in rounds as appropriate.
• Educate residents and families on exercises and safe movement; support adherence to care plan.
• Ensure compliance with facility policies and regulatory requirements; support quality initiatives.
